According to CC by-nc-sa 3.0 [[link] you are free:
to Share — to copy, distribute and transmit the work
to Remix — to adapt the work
Altohough the license obligates you attribute the original author if you've made a mod, but that doesn't mean that exclusive permission is required:
Attribution — You must attribute the work in the manner specified by the author or licensor (but not in any way that suggests that they endorse you or your use of the work).
I guess you'll just have to make your call.
I don't think there is a CC license which reflects your conditions here. You can find the terms of all CC licenses here: [link]
Even the most restrictive one: [link] allows others to redistribute (as long as they attribute the author but still they don't need to ask for permission). So in this case you could either attach suitable non-CC license or just leave a comment with your conditions on its own.
